What to Expect at Nursing and Rehabilitation

How to Prepare for Your Stay

We want you to feel as comfortable as possible during your recovery. As you prepare for your stay, consider packing the following items:

  • Loose-fitting, comfortable clothing and supportive shoes
  • Copies of your advance directive and healthcare cards and paperwork
  • List of your current medications
  • Personal care items and adaptive equipment
  • Reading materials, crossword puzzles, knitting, or other pastime activities

Your Healthcare Team

Many healthcare professionals are here to help you reach your personal recovery goals. Our expert well-coordinated care include the following:

  • Physician: Medical Director provides oversight and expert medical care
  • Nurse: Around-the-clock nursing and CNA staff provide compassionate care to ensure your medical needs are met
  • Physical Therapist: Helps you gain independence with walking, moving from sitting to standing, stairs, strengthening exercises, using adaptive mobility devices (cane, walker, or wheel chair), managing pain, and fall prevention
  • Occupational Therapist: Helps you gain independence with activities of daily living such as dressing, bathing, toileting, homemaking tasks, and home safety issues
  • Speech Therapist: Helps with communication, swallowing, and cognitive changes
  • Social Worker: Keeps you and your family updated on your rehab progress and helps with arranging discharge and continuing care, as well as connection to resources

When it is Time to Leave

From the moment you arrive a Mid Coast Senior Health, your care team will work with you on your plans to return home.

  • Our goal is to ensure a safe transition of your care.
  • Our staff will assist you with making plans and making recommendations for future needs including continuing care, meal delivery, and any other special needs.
  • We will go over discharge instructions with you and ensure that you have full understanding of medications and healthcare needs.
  • We can also assist with transportation to help you return safely home.
  • Billing is submitted to your private insurance and/or Medicare following your stay.
  • Our business office staff is available for support and questions regarding insurance and billing at any time.
